To set up an assessment or to sign up for services and classes, contact our office during regular business hours Monday, Wednesday, and Saturday from 9 a.m. to 2 p.m., Tuesday and Thursday from 9 a.m. to 7:30 p.m, and Friday from 9 a.m. to 1 p.m.

Referral Form

We’ve made it easy for you to complete the forms you need by using a computer (these forms cannot be filled out using a phone). Depending on the web browser, you can complete the form online, download it, and email it back to us, or download it, print it out, and bring the completed form to our office.
